Bacterial Meningitis Vaccination Information

Effective Jan. 1, 2012, all new students under the age of 30 are required to submit evidence of immunization against bacterial meningitis.

Proof of immunization will be required before students are permitted to register for classes. The vaccination requirement applies to:

  • All new first year (1L) students
  • All new transfer and visiting students
  • All new students auditing a course
  • All returning Texas Wesleyan students who have experienced a break in their Texas Wesleyan enrollment of at least one fall or spring semester

Required Forms

At least one of the following must be faxed, mailed or submitted in person to the admissions office prior to registration for classes:

  1. Certification from a physician or clinic that the student has been vaccinated during the five-year period immediately preceding and at least 10 days prior to the first day of class (must include the month, day, and year of immunization). Click here to download the form.
  2. An immunization record from a state or local health authority or an official record received from school officials (not older than 5 years and must include the month, day, and year the immunization was administered).

Students may fill out an extension request form if they need more time to fulfill the vaccination requirement.

Students who fail to submit the required bacterial meningitis documents by the deadline will not be permitted to register for classes.

Texas Wesleyan University will maintain the above information in accordance with Family Education Rights and Privacy Act (FERPA) regulations and the Health and Insurance Portability and Accountability Act.

Vaccination Exemptions

Students are exempt from the bacterial meningitis vaccination if:

  • They are 30 years old by the date of matriculation and/or class registration.
  • They have an affidavit or certificate from a licensed U.S. physician stating the vaccination is injurious to the student's health.
  • They have attached a notarized conscientious exemption form from the Texas Department of Health and Services due conscience or religious beliefs.
